What is actually going wrong

Leaking when you sneeze is common. It is not normal, and it is not something you have to live with. Pelvic floor rehab resolves most cases.

Pregnancy and delivery load the pelvic floor, abdominal wall and spine more than almost anything else a body does. These tissues recover — but recovery is a rehabilitation process, and it is one almost nobody is offered. The result is a large number of women quietly managing symptoms for years that would respond to a structured 12-week programme.

The problem

The pelvic floor loses coordination with the breath and deep core after delivery.

See a doctor today, not a physiotherapist, if you have any of these

  • Heavy vaginal bleeding, fever, or foul-smelling discharge postnatally
  • Severe or sudden pelvic pain
  • New or worsening incontinence with leg weakness or numbness
  • Any bleeding after menopause — needs medical review, not physiotherapy first

These are uncommon, but they need medical assessment rather than physiotherapy. If you are not sure, call us on +91 86193 28981 and we will tell you honestly where to go.

Cause and symptoms

Why it started, and how it shows up

Most people arrive having been told only what is wrong on a scan. What matters clinically is which of these is driving your particular case.

Common causes

  • Pelvic floor weakness or over-activity following pregnancy and delivery
  • Diastasis recti — separation of the abdominal muscles
  • Pregnancy-related pelvic girdle and low back pain
  • Post-caesarean scar restriction and core inhibition
  • Menopause-related changes in tissue and bone health
  • Returning to running or lifting without a graded postnatal build-up

What patients describe

  • Leaking urine when coughing, sneezing, laughing or running
  • A heaviness or dragging sensation in the pelvis
  • A visible doming or gap along the midline of the abdomen
  • Pelvic or low back pain during or after pregnancy
  • Pain with intercourse
  • Difficulty controlling wind or urgency
Our approach

What we actually do — step by step

No two programmes are identical, but every one is built from these components in this order.

01

Private, unhurried assessment with a female therapist

Every women's health appointment is with a female physiotherapist in a private room, with a chaperone available on request. Nothing is examined without your explicit consent, and you can decline any part at any point.

02

Pelvic floor assessment done correctly

A meaningful proportion of women perform Kegels incorrectly, and a subset have an over-active floor that gets worse with them. We establish which problem you actually have before prescribing anything.

03

Antenatal preparation

Pelvic girdle pain management, safe exercise through each trimester, breathing and positioning for labour.

04

Postnatal restoration

Diastasis assessment, deep core reconnection, scar mobilisation and a graded return-to-running programme — most guidance suggests waiting until at least 12 weeks postpartum and passing specific load tests.

05

Menopause and bone health

Progressive resistance and impact loading for bone density, plus continence support.

Recovery timeline

How long it usually takes

A typical progression, not a promise. Your plan is set after assessment and re-checked every two weeks — if the line is flat, we change the plan.

  1. Week 1

    Assess & educate

Identify the actual mechanism; correct technique

You should be able to: You can perform a correct, verified pelvic floor contraction

  2. Weeks 2–6

    Reconnect

Deep core and pelvic floor coordination with breath

You should be able to: Noticeably fewer leaking episodes

  3. Weeks 6–12

    Load

    Strength under real-life load

You should be able to: Lifting your child, coughing, and jumping without leaking

  4. Weeks 12+

    Return

Return to running, gym or sport

You should be able to: Cleared on a load-test battery with a maintenance plan

Individual results vary. How long you have had the symptoms, other health conditions, and how consistently you do the home programme all change this timeline — often substantially. We will give you a personalised estimate in writing after your assessment, and revise it honestly if progress is slower than expected.

Expected results

What the numbers typically look like

Typical ranges from clinical evidence and our own caseload. Your results depend on how long you have had the problem, what else is going on, and how consistently you do the programme.

~70%

Stress incontinence improved or resolved

With supervised pelvic floor training over 12 weeks

12 weeks

Programme length for meaningful change

Pelvic floor muscle is muscle — it follows the same adaptation timeline

12 weeks+

Earliest evidence-guided return to running

Postpartum, and subject to passing load tests

Why patients choose this programme

  • Female therapists, private rooms, and consent-led examination throughout
  • Addresses symptoms most women were told to simply accept
  • Safe, structured return to exercise instead of guessing
  • Evening appointment slots for working mothers
Questions

Women's Health: what people ask us

If your question is not here, send it on WhatsApp. A physiotherapist answers, usually within the hour during centre hours.

  • It is common — roughly a third of women experience it — but common is not the same as normal, and it is not something you have to accept. Supervised pelvic floor training resolves or substantially improves the majority of stress incontinence cases.
